Why Dictate?
Preventative medicine: Handwritten notes (and possible illegibility) are potential pitfalls in reimbursement documentation and litigation.
Patient care: Continuation of patient care in the absence of the primary physician is based on the documentation in the chart: if it is not documented, the patient’s care could potentially be compromised. Further, notes that are shared with other physicians caring for the patient should be complete, accurate and legible to assist in the overall care and well-being of the patient.
With managed care companies imposing limitations on treatment and reimbursement, more narratives are being required to justify the physician’s actions and fees.
